* Medical records, e.g.: Discharge summary from ADF Garrison GP, Current medications, Specialist reports
* A list of questions for your doctor

How to Transfer Your Medical History:
1. Request a Discharge Summary from your ADF Garrison Health Centre
2. Ask for your full records (direct or via FOI)
3. Provide your new GP with:
Your PMKeyS ID and Service Number, A signed records transfer form
4. Bring printed or digital copies of your health files if possible
We Can Help
* Our admin team can assist with all DVA paperwork and record transfers

DVA Gold Card
* Covers all medical conditions, regardless of service relation
* Includes GP, specialists, hospital, allied health, medications (RPBS), and more
DVA White Card
* Covers only service-related conditions

Ongoing Care from Home
* After your first in-person visit, all future telehealth appointments are DVA-covered (White or Gold Card)
* Gold Card: all conditions
* White Card: accepted conditions + mental health
* 100% bulk billed, as long as you're in Australia
Overseas Telehealth Not Covered
DVA telehealth is only valid within Australia. Overseas consultations are not eligible for DVA billing.

Complete Veteran Health Opening Hours:
* Monday to Friday: 8:00 AM – 6:00 PM
* Saturday: 9:00 AM – 1:00 PM
* Sunday & Public Holidays: Closed

All DVA Visits Are Bulk Billed
* All consultations are 100% bulk billed under your DVA White or Gold Card.
* We cover your Permanent Impairment (PI) paperwork and all standard DVA assessments.

If You Ever Need to Pay
* For rare services not covered automatically (e.g. some specialist procedures), we'll notify you in advance.
* We handle DVA prior approval requests and submit forms on your behalf.
* Most services are approved — we’ll support you every step of the way.
